How do I make an appointment at the clinic?

Email us at [email protected] with your contact information. Alternatively, you may call us at (312) 781-0696. We will generally answer your inquiry within one working day. Always call 911 if you are experiencing a medical emergency.

Who do I speak with about appointments, prescriptions, and other non-urgent matters?

Email us at [email protected] or call (312) 781-0696 . For most prescription refills, you may also ask your pharmacy to send an electronic refill request to our office. Please allow 1 business day for prescription approvals. Should the patient’s insurance company deny coverage for a particular medication, we “appeal” as quickly as possible to attempt to resolve the situation. The initial “appeal” is known as a “prior authorization.”

Can you prescribe medications to me before I come in for my evaluation appointment?

We are not able to provide treatment until they are seen in our office and under our care.

What is your cancellation policy?

You may cancel and/or change your appointment up to 24 hours in advance on weekdays, and 48 hours over the weekend for Monday appointments.

How long are your appointments?

Typically, the initial evaluation is 55-60 minutes and follow-up appointments are 25-30 minutes. We book longer appointments when necessary or by request from our patients.

Do you offer “second opinions?”

We do consultations for other physicians when requested. Our process is the same as an initial evaluation. In addition, we are able to send our full electronic medical record notes to your treating physician and/or speak with them by phone with recommendations.
